Breakaway Community Services is a multifunction treatment agency that provides a range of outpatient, day treatment and outreach programs. We are strongly committed to providing innovative community services from a harm reduction, trauma-informed perspective. We develop programs that are accessible, barrier free and open to all community members.
*Core Team: *The Family and Youth initiative (FYI) is an outpatient program developed to meet the needs of youth, aged 12 to 25, and their family members and friends who have substance use concerns.
The program provides services to both the youth with substance use issues and their family. The program can work with parents, guardians, or other family members whether or not the youth is willing to engage in counselling. The program also provides support and services for youth who are affected by parents or guardians with substance use challenges. Services offered include: Assessments and Referrals, Individual and Family Therapy, School Outreach, Outreach Counselling and Community Education and Presentations.
The Therapist on the FYI team provides client assessments, case management, individual, family and group counselling to youth aged 12-25 and their families. In addition to individual and family work, the Therapist works collaboratively with external professionals and service providers to address substance use concerns, increase knowledge and capacity to provide substance use informed services to youth and families, and delivers educational presentations and workshops.
*Associated Programs:* The Youth and Family Counsellor will work in collaboration with our other youth programs. These include our Pieces to Pathways (P2P) and Toronto Opiate Support Teams (TOST). As such, knowledge and competencies working with LGBTTQQ2SIA youth 16 to 29, and youth who are using or at risk of using opiates will be an asset to the position. The Youth and Family Counsellor will work with the agency as a whole to provide guidance when working with youth and their families.
*E*_*S*_*SENTIAL DUTIES AND RESPONSIBILITIES:*
*Client Service*
* Contribute to the program team and agency in the delivery of substance use and addiction services for youth and families from a harm reduction framework
* Deliver responsive, accessible, and flexible service from a harm reduction and trauma informed approach
* Complete intakes and comprehensive assessments and treatment planning
* Work with a caseload of clients to provide counselling/therapy and case management services, including facilitating referrals, systems advocacy and navigation
* Provide counselling, parenting support, consultation and assistance to families and significant others who may be impacted by the substance use of an individual who may or may not be receiving services
* Facilitate group programming
* Provide harm reduction and overdose education and responses
* Collaborate with internal and external health and social service providers to coordinate and deliver services.
* Provide substance use related education and health promotion programs in the form of presentations and other appropriate interventions with parents, children and youth in the community, schools and other venues.
