JOB PURPOSEThe LPN provides Primary Health Care services, Illness/Injury Prevention, Chronic Disease Management, and Community Development services, using a population health approach in an interdisciplinary team of health providers. In keeping with the Community Health Centre framework, and the Community Health Program Standards, the service model is based on the principles of primary health care with the primary goal of supporting communities in meeting their health care needs.As a member of the interdisciplinary team, provides quality patient care within their scope of practice to patients and families. Performs duties and responsibilities ( clinical and non clinical) under the direction of the Registered Nurse, Physicians, Nurse Practitioners and/or other primary care providers. QUALIFICATIONSActive registration with the Association of New Brunswick Licensed Practical Nurses (ANBLPN).Current BCLS certificationMedication Administration Course and Adult Physical Assessment CertificationDemonstrated ability to work well under the direction of the Registered Nurse/Primary Care Providers and respond to rapidly changing priorities.Good work and attendance recordDemonstrated communication, interpersonal and team-building skills.Certified in phlebotomy in centers where phlebotomy is performedPhysically able to perform assigned duties.Ability to attend work on a regular basis.Ability to perform the duties of the position.Good work record.KEY RESPONSIBILITIESThe LPN reports to the Community Health Centre Manager Patient CareManages phlebotomy and/or other specimens for collection, following regional procedures with accurate documentationCollects other specimen as ordered, appropriately following regional procedures and documenting accuratelyPrepares specimen for transport to the lab, following policy and procedures and Transportation of Dangerous Goods guidelinesAppropriately supports other screening and monitoring processes such as diabetic screening > 40 years, colorectal screening, etc.Participates and collaborates with the team in the development and implementation of the patient focused care plan and self-management plan.Delivers safe and effective patient care.Assists the patient to the exam room, determines the reason for visit and appropriately assesses/prepares the patient for the primary care provider. This includes appropriately disrobing and draping the patient as necessary, completing the vital signs, weight, height etc, with accurate documentation. Provides appropriate support supplies and/or equipment. After the visit is completed, prepares the room for the next patient. Documents nursing care according to nursing standards and Horizon Health Network policies and procedures.Identifies and reports pertinent information to the Registered Nurse or Primary Care Provider.Communication/TeamworkPromotes effective inter-departmental communication.Communicates effectively with patients, families and members of the interdisciplinary team.Communicates clinical/administrative concerns to Centre/Clinic Manager or designatePromotes positive relationships with patients, families, and communities. Works collaboratively with members of the interdisciplinary team.Assists in developing and maintaining effective relationships with outside agencies and the community.Assists with identifying, planning, and implementing health promotion programs based on determined need, in collaboration with individuals, groups and other health care professionals. EducationAttends in services and continuing education programs for self-development and participates in formal educational programs.Assumes responsibility for maintaining clinical competence.Maintains annual certification in skills / programs as required.Shares expertise with peers and others.Quality ManagementParticipates in quality management activities at the facility level such as documentation of incidents, cost-effective use of equipment and supplies, and activities as specific to th
