The creation of a provincial health authority was announced following recommendations from Health Accord NL.NL Health Services is responsible for delivering health and community services to more than 528,000 people living in Newfoundland and Labrador and employs over 21,000 people.NLHealthServices.ca Job Summary This position works within the Nurse Practitioner's (NP) scope of practice in the category of practice of Family/All Ages. This is an advanced practice role providing professional, analytical and an autonomous comprehensive range of health services to clients and patients including but not limited to health assessments and diagnosis of diseases, disorders, injuries and conditions; treating and managing acute and chronic illnesses and identifying health risks and needs within a holistic model. In this role the nurse practitioner performs all activities utilizing a harm reduction approach within the primary health care framework.In keeping with the organizations' vision of "Healthy People - Healthy Communities", Eastern Health is pleased to support its Primary Health Care Harm Reduction Team in achieving its goals of providing exceptional and high-quality care to vulnerable clients within the St. John's-Metro region, and beyond.The PHC Harm Reduction Team mandate is to provide low barrier access to harm reduction focused, primary health care services for individuals who are homeless or precariously housed. Many of these individuals are navigating complex mental health and/or substance use conditions and often experience marginalization because of their circumstances; many have trouble accessing traditional health care services.The nurse practitioner is a member of a harm reduction team of health professionals including, but not limited to, registered nurses, pharmacist, occupational therapist, community outreach workers, social worker, peer support workers and others, working across programs and services to provide the best possible care. Health services in this context are provided in a variety of ways including street reach and mobile clinics, home visits, PHC walk-in clinics, community partnerships, and acute care partnerships.The nurse practitioner, as a member of the team, provides for the presenting needs of the client and includes all routine primary health care and treatment as well as Hepatitis C, HIV, latent TB, STBBI testing and treatment, mental health single session counselling, client education, and is a satellite site for provision of SWAP supplies.
