Job Summary:
We are seeking a dedicated and knowledgeable Registered Dietitian to join our healthcare team.
The Registered Dietitian (RD) is a member of the centralized WestView Primary Care Network team, providing comprehensive dietary support and obesity counseling through a referral-based process. The RD plays a pivotal role in promoting health and wellness through tailored dietary interventions, education, and counseling services.
The RD works collaboratively with member clinics and the centralized obesity program, and directly impacts patient outcomes in a dynamic primary care environment.
Key Responsibilities:
· Conduct comprehensive nutritional assessments for patients across diverse demographics, considering medical history, dietary habits, and lifestyle factors. Work with Quality Improvement and Evaluation Facilitator to implement dashboard / scorecard for quality reporting
· Develop personalized dietary plans and interventions to address individual health needs, focusing on weight management, chronic disease prevention, and overall wellness.
· Provide ongoing counseling and support to empower patients in making sustainable dietary changes, utilizing evidence-based strategies and behavioral modification techniques.
· Develop and deliver educational materials, workshops, and group sessions on nutrition, healthy eating habits, and weight management within member clinics and community settings.
· Participate in quality improvement initiatives to enhance the delivery of nutrition services within primary care settings, including developing protocols, tracking outcomes, and implementing best practices.
Requirements:
- Bachelor of Science with a major in Nutrition.
- Completion of an internship program or practicum training program accredited by the Dietitians of Canada
- Familiarity with electronic medical records systems and proficiency in utilizing technology to support patient care and documentation
- Active or eligible for registration with the College of Dietitians of Alberta (CDA).
- Experience working with diverse patient populations, including pediatric, geriatric, and culturally diverse communities
